HIPAA Notice of Privacy Practices

Who We Are
Women’s Preventive Care (“the Practice,” “we,” “us,” or “our”) is a women’s health clinic located at 1011 Virginia Dr, Suite 102, Orlando, FL 32803. This Notice applies to all Protected Health Information (“PHI”) created or received by the Practice.
Our Commitment
We are required by law to maintain the privacy of your PHI, to provide you with this Notice of our legal duties and privacy practices regarding your PHI, and to notify you in the event of a breach of unsecured PHI. We must follow the terms of the Notice currently in effect.
How We May Use and Disclose Your Health Information
We may use and disclose your PHI for the following purposes without your written authorization:
- Treatment. To provide, coordinate, or manage your healthcare and related services. For example, sharing information with another clinician involved in your care.
- Payment. To bill and collect payment for services from you, your insurer, or another responsible payer.

- As Required by Law. When required by federal, state, or local law (e.g., reporting communicable diseases, child or elder abuse, or responding to a court order).
- Public Health Activities. To public health authorities for activities such as preventing or controlling disease, reporting adverse events, and product safety.
- Health Oversight. To agencies authorized to audit, investigate, or license healthcare providers.
- Appointment Reminders & Health-Related Communications. To remind you of appointments or to inform you of treatment alternatives or services that may be of interest to you.
Uses and Disclosures Requiring Your Written Authorization
Most uses and disclosures of psychotherapy notes, marketing communications, and the sale of PHI require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.
Online Information and Advertising Technologies
Information you submit through our website’s scheduling, intake, or contact forms that relates to your health is treated as PHI and is stored in our HIPAA-compliant systems. Anonymized website analytics (such as which pages you visited) collected by tools like Google Analytics or Google Ads are kept separate from PHI and are not used to identify you as a patient. We do not share PHI with advertising platforms.
Your Rights Regarding Your Health Information
- Right to Inspect and Copy. You may request to inspect and obtain a copy of your medical record.
- Right to Amend. You may request that we amend information you believe is incorrect or incomplete.
- Right to an Accounting of Disclosures. You may request a list of certain disclosures of your PHI.
- Right to Request Restrictions. You may request limits on how we use or disclose your PHI for treatment, payment, or operations.
- Right to Confidential Communications. You may request that we contact you in a specific way or at a specific location.
- Right to a Paper Copy of This Notice. You may request a paper copy at any time, even if you have agreed to receive it electronically.
- Right to Be Notified of a Breach. You will be notified following a breach of your unsecured PHI.
How to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with us at the contact information below or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
Changes to This Notice
We reserve the right to change the terms of this Notice and to make the new Notice provisions effective for all PHI we maintain. Updated Notices will be posted on our website and made available at the practice.
