Terms of Service
Welcome to Women’s Preventive Care Clinic. These Terms of Service (“Terms”) govern your use of the womenspreventivecare.com website (the “Site”) and the services we provide. By accessing the Site or engaging with our services, you agree to these Terms. If you do not agree, please do not use the Site or our services.
1. About Us
Women’s Preventive Care Clinic, PLLC is a Florida professional limited liability company. Our practice is a cash-pay concierge women’s health clinic operated by Luciana Svilpa, WHNP-BC, located at 1011 Virginia Drive, Suite 102, Orlando, FL 32803. References to “we,” “us,” “our,” and “the clinic” throughout these Terms refer to Women’s Preventive Care Clinic, PLLC.
2. Eligibility
The Site and our services are intended for individuals 18 years of age or older located in the United States. By using the Site or our services, you represent that you meet these eligibility requirements. We do not knowingly provide services to individuals under 18.
3. Medical Disclaimer
The information provided on this Site is for general educational and informational purposes only. It is not a substitute for professional medical advice, diagnosis, or treatment.
Use of this Site does not establish a provider-patient relationship between you and Women’s Preventive Care Clinic or Luciana Svilpa, WHNP-BC. A provider-patient relationship is formed only after you have completed an in-person or telehealth consultation, signed the required intake and consent forms, and been formally accepted as a patient of the practice.
Always seek the advice of a qualified healthcare professional regarding any medical condition, symptom, or treatment decision. Never disregard professional medical advice or delay seeking it because of something you have read on this Site. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
Content on this Site, including but not limited to information about hormone replacement therapy, metabolic programs, perimenopause and menopause care, sexual health, and preventive wellness, is provided for educational purposes and is not personalized medical advice. Treatment decisions are made on an individual basis after a full clinical evaluation.
4. Services and Cash-Pay Model
Women’s Preventive Care Clinic is a cash-pay concierge practice. We do not bill insurance directly. Patients are responsible for payment in full at the time services are rendered. Upon request, we can provide an itemized superbill that you may submit to your insurance carrier for possible reimbursement; however, reimbursement is not guaranteed and is determined solely by your insurance plan.
Pricing for consultations, procedures, and treatment programs is published on the Site or provided in writing prior to service. Pricing is subject to change; the price quoted at the time of booking is the price that will apply to that visit.
5. Booking, Cancellation, and No-Show Policy
Booking
Appointments may be scheduled through the Site, by phone, or through the patient portal. A valid credit or debit card may be required to hold an appointment.
Cancellation
We require at least 24 hours’ notice to cancel or reschedule an appointment without charge. Cancellations made less than 24 hours before the scheduled appointment time will be charged the full appointment fee.
No-Shows
Patients who do not arrive for a scheduled appointment and have not provided notice will be charged the full appointment fee. Repeated no-shows may result in dismissal from the practice.
Late Arrivals
If you arrive more than 15 minutes late for a scheduled appointment, the clinic reserves the right to reschedule your visit at the discretion of the clinician.
6. Payment and Refund Policy
Payment for services is due at the time of service unless other arrangements have been made in writing. We accept major credit cards and other payment methods as published on the Site.
All payments for services rendered are non-refundable. Once a service has been provided — including consultations, procedures, treatment plans, lab orders, and prescription services — the associated payment will not be refunded. Membership fees, program enrollment fees, and prepaid service packages are non-refundable once any portion of the service has been initiated.
Disputes regarding charges must be raised in writing within 30 days of the charge by contacting us at info@womenspreventivecare.com.
7. Patient Responsibilities
To receive safe and appropriate care, patients agree to:
- Provide accurate, complete, and current health information
- Disclose all medications, supplements, allergies, and relevant medical history
- Follow agreed-upon treatment plans and attend scheduled follow-up visits
- Notify the clinic promptly of any changes in health status, adverse reactions, or new symptoms
- Communicate respectfully with clinic staff
- Comply with prescription, laboratory, and imaging requirements as recommended by the clinician
8. Communications with the Clinic
The clinic communicates with patients through several channels, including the secure patient portal (SigmaMD), phone, email, SMS text messaging (with consent), and the website chat widget. Clinical communications — including questions about treatment, lab results, medications, and care plans — must be conducted through the secure patient portal. Standard SMS, email, and social media are not secure channels and are limited to scheduling and general logistics.
Detailed terms governing SMS text messaging are provided in Section 13 (SMS Communications) below.
